Your 5-Step Decision Journey

Follow these steps to choose the right plan for 2026.

Get the Big Picture

Read the overview to understand the three plan options. Each has different costs, networks, and coverage levels.

Key difference: Bronze and Silver use HPN, Gold uses the broader PPO.

Check Whether Your Providers Are In-Network

This is the most important decision factor.

Understanding the HPN Network

Bronze and Silver ONLY work with HPN providers. HPN is built around Duke Health.

What’s Included:

100+ Duke Primary Care locations
2,000+ Duke specialists in every field
Duke hospitals and cancer centers
Select Duke-affiliated providers

⚠️ NOT Included:

UNC Health
WakeMed (except emergencies)
Rex Hospital (except emergencies)

Bottom Line: If you use UNC, WakeMed, or Rex, you need Gold.

HPN (Bronze/Silver)
  • All Duke Health providers
  • 100+ primary care locations
  • 2,000+ specialists
  • Nationwide emergencies

NOT included: UNC, WakeMed, Rex

PPO (Gold)
  • All Duke Health
  • UNC Health system
  • WakeMed hospitals
  • Rex Hospital
  • All NC providers
  • Nationwide coverage
  • Out-of-network (50%)

Think About Your Medical Needs

Consider:

  • How often do you see doctors?
  • Do you have regular specialists?
  • Are your doctors at Duke, UNC, or elsewhere?
  • Do you take regular medications?
  • Any surgeries or procedures expected?

Bring Questions to the Broker Visit

Broker Q&A Sessions

  • Questions about costs

Understanding Key Terms

Insurance terminology simplified for real-world understanding.

Real Numbers

What Will This Actually Cost Me?

See how each plan performs in common medical situations. Your total cost has two parts:

1. Premium Buy-Up — What you pay from your paycheck to have insurance (26 paychecks/year)

2. Out-of-Pocket Costs — What you pay when you use covered services (deductible, coinsurance, copays)

Note: Premium buy-up does NOT count toward your out-of-pocket maximum. These are separate buckets.

Routine Year

Annual physical, 2 doctor visits, prescription refills, no major issues.

Bronze $150
Premium Buy-Up $0
Copays for Covered Services ~$150
Total Annual Cost $150 Best
Silver $1,320
Premium Buy-Up $1,170
Copays for Covered Services ~$150
Total Annual Cost $1,320
Gold $3,198
Premium Buy-Up $3,048

Total Annual Cost $3,198

Having a Baby

Prenatal visits, ultrasounds, delivery, postnatal care. Typical costs: ~$10,000.

Bronze $5,800
Premium Buy-Up $0
Covered Services (Deductible + Coinsurance) ~$5,800
Total Annual Cost $5,800
Silver $5,570

Total Annual Cost $5,570 Best
Gold $7,448
Premium Buy-Up $3,048

Major Surgery

Unexpected surgery, hospital stay, specialists, rehab. Total costs: $50,000+.

Bronze $9,500
Premium Buy-Up $0
OOP Max (Deductible, Coinsurance, Copays) $9,500
Total Annual Cost $9,500
Silver $8,170
Premium Buy-Up $1,170
OOP Max (Deductible, Coinsurance, Copays) $7,000
Total Annual Cost $8,170 Best
Gold $10,048
Premium Buy-Up $3,048
OOP Max (Deductible, Coinsurance, Copays) $7,000
Total Annual Cost $10,048
Understanding the Trade-Offs

Bronze wins for: Healthy years with minimal care. You save on premiums but pay more if you need significant services. Best if you’re comfortable with Duke Health network.

Silver wins for: Moderate to high medical needs. The $1,170 annual premium is offset by lower deductible and out-of-pocket maximum. Even in worst-case scenarios, Silver caps at $8,170 total vs Bronze at $9,500.

Gold’s value: Network flexibility trumps cost. While Gold costs $10,048 in a major medical year (vs Silver’s $8,170), it’s the ONLY option if you need UNC Health, WakeMed, or Rex Hospital. Choose Gold for provider access, not cost savings.

Key Insight: Silver delivers the best financial protection in moderate-to-high cost scenarios. The $1,170 annual premium saves you $2,500 in maximum out-of-pocket costs compared to Bronze ($7,000 vs $9,500). Gold provides unmatched network freedom but costs about $1,878 more than Silver annually when hitting the OOP max.

Critical: HPN Network Limitation

Bronze uses the High Performance Network (HPN), built around Duke Health. You MUST use HPN-participating providers to receive in-network benefits.

What’s included: All Duke Health providers, 100+ Duke Primary Care locations, 2,000+ Duke specialists

What’s NOT included: UNC Health, WakeMed (except emergencies), Rex Hospital (except emergencies)

Action Required: Verify your doctors participate in HPN before choosing Bronze. Use the Blue Cross provider search or call 1-800-359-2422.

Choose Bronze If You:

  • Use Duke Health providers or are willing to switch to Duke-affiliated doctors
  • Are generally healthy and see doctors infrequently for routine care only
  • Want no paycheck deduction and are comfortable with higher out-of-pocket costs if unexpected medical needs arise
  • Value the free telehealth benefit for minor illnesses and questions

Same HPN Network as Bronze

Silver uses the same High Performance Network (HPN) as Bronze. You must use Duke Health providers for in-network benefits.

What’s included: All Duke Health providers, 100+ Duke Primary Care locations, 2,000+ Duke specialists

What’s NOT included: UNC Health, WakeMed (except emergencies), Rex Hospital (except emergencies)

If you need UNC, WakeMed, or Rex: You must choose Gold (PPO). Silver will NOT cover these providers except for emergencies.

Choose Silver If You:

  • Use Duke Health providers and expect moderate to frequent medical care
  • See specialists regularly — unlimited $70 visits vs 4× $135 then 30% in Bronze
  • Take regular medications — lower copays ($8 vs $15 generic, $25 vs $35 brand)
  • Want better financial protection — $7,000 OOP max vs $9,500 saves $2,500
  • Can afford $45/paycheck — often offset by lower costs when you need care

What if I don’t enroll by December 15?

Can I stay in Bronze?

Yes! Bronze is the default plan. No action needed if you want to stay in Bronze.

How does payroll deduction work?

Bronze: $0 per paycheck (no deduction)

Silver: $45.04 deducted per biweekly paycheck (pre-tax)

Gold: $117.24 deducted per biweekly paycheck (pre-tax)

All deductions are taken pre-tax, which reduces your taxable income.

Can I change plans mid-year?

Only if you have a qualifying life event, such as:

  • Marriage or divorce
  • Birth or adoption of a child
  • Loss of other health coverage (e.g., spouse’s job loss)
  • Change in dependent eligibility

You must notify HR within 30 days of the qualifying event.

How do I verify my doctors are in HPN?

Online: Use the Blue Cross provider search tool

By phone: Call Blue Cross at 1-800-359-2422

Ask your doctor: “Are you in the Blue Cross Blue Shield High Performance Network?”

Quick tip: All Duke Health providers are in HPN. UNC Health, WakeMed, and Rex Hospital are NOT in HPN (except for emergencies).

What if my doctor isn’t in HPN?

You have two options:

Option 1: Choose Gold (PPO) for $117/paycheck. Gold covers ALL providers nationwide, including UNC Health, WakeMed, Rex Hospital, and any other Blue Cross participating provider.

Option 2: Find a new doctor in HPN and choose Bronze ($0) or Silver ($45/check). All Duke Health providers are in HPN.

When will I get my insurance card?

Physical cards: Mailed by January 15, 2026

Digital access: Available January 1, 2026 through the Blue Cross mobile app or website

You can use digital ID cards at doctor’s offices and pharmacies while waiting for your physical card.

What if I need care before January 1?

Your current 2025 coverage continues through December 31, 2025. Your new 2026 plan begins January 1, 2026.

There is no gap in coverage.

Can I add family members?

Yes. Contact Gary Nance, Christopher Ingram, or Joanne Noel to add dependents (spouse, children).

Family coverage costs more than employee-only coverage. They’ll provide you with the family rates for each plan.

What if I turn 65 this year?

If you’re approaching Medicare eligibility (age 65), contact Gary Nance to discuss coordination options.

You may:

  • Stay on the employer plan
  • Transition to Medicare
  • Coordinate both coverages

Gary can provide neutral guidance on what works best for your situation.
