What Will This Actually Cost Me?
See how each plan performs in common medical situations. Your total cost has two parts:
1. Premium Buy-Up — What you pay from your paycheck to have insurance (26 paychecks/year)
2. Out-of-Pocket Costs — What you pay when you use covered services (deductible, coinsurance, copays)
Note: Premium buy-up does NOT count toward your out-of-pocket maximum. These are separate buckets.
Routine Year
Annual physical, 2 doctor visits, prescription refills, no major issues.
Having a Baby
Prenatal visits, ultrasounds, delivery, postnatal care. Typical costs: ~$10,000.
Major Surgery
Unexpected surgery, hospital stay, specialists, rehab. Total costs: $50,000+.
Bronze wins for: Healthy years with minimal care. You save on premiums but pay more if you need significant services. Best if you’re comfortable with Duke Health network.
Silver wins for: Moderate to high medical needs. The $1,170 annual premium is offset by lower deductible and out-of-pocket maximum. Even in worst-case scenarios, Silver caps at $8,170 total vs Bronze at $9,500.
Gold’s value: Network flexibility trumps cost. While Gold costs $10,048 in a major medical year (vs Silver’s $8,170), it’s the ONLY option if you need UNC Health, WakeMed, or Rex Hospital. Choose Gold for provider access, not cost savings.
Key Insight: Silver delivers the best financial protection in moderate-to-high cost scenarios. The $1,170 annual premium saves you $2,500 in maximum out-of-pocket costs compared to Bronze ($7,000 vs $9,500). Gold provides unmatched network freedom but costs about $1,878 more than Silver annually when hitting the OOP max.

Annual wellness visits and the first three sick visits to your selected Primary Care Provider have $0 copay. After the first three visits: $45 copay per PCP visit.
All Preventive Care at No Cost
Annual physicals, immunizations, cancer screenings, mammograms, colonoscopies, well-child visits. No deductible, no copay, no cost-sharing when coded as preventive.
Free 24/7 Nurse Hotline
Direct access to registered nurses for health questions. Get guidance on whether to seek care in person or treat at home. Helps reduce unnecessary urgent care and ER visits.

What’s included: All Duke Health providers, 100+ Duke Primary Care locations, 2,000+ Duke specialists
What’s NOT included: UNC Health, WakeMed (except emergencies), Rex Hospital (except emergencies)
Action Required: Verify your doctors participate in HPN before choosing Bronze. Use the Blue Cross provider search or call 1-800-359-2422.
Choose Bronze If You:
- Use Duke Health providers or are willing to switch to Duke-affiliated doctors
- Are generally healthy and see doctors infrequently for routine care only
- Want no paycheck deduction and are comfortable with higher out-of-pocket costs if unexpected medical needs arise
- Value the free telehealth benefit for minor illnesses and questions

Premium Option • Maximum Flexibility
Gold’s Unique Advantage
Full PPO Network Access
See ANY provider nationwide — Duke, UNC, WakeMed, Rex, and all Blue Cross NC participating providers. No network restrictions.
Out-of-Network Coverage
If you see a non-Blue Cross provider, Gold still provides coverage at reduced benefits (60% coinsurance after deductible). Bronze/Silver provide no coverage for out-of-network except emergencies.
No Provider Changes Required
Keep your existing doctors at UNC, WakeMed, Rex, or any other system. Gold is the only plan that allows you to stay with non-Duke providers without penalty.

How to Enroll
All employees are automatically enrolled in Bronze at no cost. To choose Silver or Gold, contact HR or Gary Nance by December 15.

How do I enroll in Silver or Gold?

They’ll help you complete the election form and authorize payroll deduction.
What if I don’t enroll by December 15?
You’ll automatically be enrolled in Bronze (HPN-45) at $0 per paycheck. This is the default plan for all employees.
Can I stay in Bronze?
Yes! Bronze is the default plan. No action needed if you want to stay in Bronze.
How does payroll deduction work?
Bronze: $0 per paycheck (no deduction)
Silver: $45.04 deducted per biweekly paycheck (pre-tax)
Gold: $117.24 deducted per biweekly paycheck (pre-tax)
All deductions are taken pre-tax, which reduces your taxable income.
Can I change plans mid-year?
Only if you have a qualifying life event, such as:
- Marriage or divorce
- Birth or adoption of a child
- Loss of other health coverage (e.g., spouse’s job loss)
- Change in dependent eligibility
You must notify HR within 30 days of the qualifying event.
How do I verify my doctors are in HPN?
Online: Use the Blue Cross provider search tool
By phone: Call Blue Cross at 1-800-359-2422
Ask your doctor: “Are you in the Blue Cross Blue Shield High Performance Network?”
Quick tip: All Duke Health providers are in HPN. UNC Health, WakeMed, and Rex Hospital are NOT in HPN (except for emergencies).
What if my doctor isn’t in HPN?
You have two options:
Option 1: Choose Gold (PPO) for $117/paycheck. Gold covers ALL providers nationwide, including UNC Health, WakeMed, Rex Hospital, and any other Blue Cross participating provider.
Option 2: Find a new doctor in HPN and choose Bronze ($0) or Silver ($45/check). All Duke Health providers are in HPN.
When will I get my insurance card?
Physical cards: Mailed by January 15, 2026
Digital access: Available January 1, 2026 through the Blue Cross mobile app or website
You can use digital ID cards at doctor’s offices and pharmacies while waiting for your physical card.
What if I need care before January 1?
Your current 2025 coverage continues through December 31, 2025. Your new 2026 plan begins January 1, 2026.
There is no gap in coverage.
Can I add family members?
Yes. Contact Gary Nance, Christopher Ingram, or Joanne Noel to add dependents (spouse, children).
Family coverage costs more than employee-only coverage. They’ll provide you with the family rates for each plan.
What if I turn 65 this year?
If you’re approaching Medicare eligibility (age 65), contact Gary Nance to discuss coordination options.
You may:
- Stay on the employer plan
- Transition to Medicare
- Coordinate both coverages
